At CHOR Youth & Family Services, our community-based programs are designed to meet people where they are, offering compassionate, practical services that help build stability, connection, and hope. Whether you’re a young parent, a child in need of support, or an adult working toward recovery, we’re here to walk alongside you.

From in-home support to school partnerships and peer-led drop-in centers, we focus on creating safe spaces and meaningful relationships that promote long-term wellness. FOSTER CARE &
SPECIALIZED FOSTER CARE

(Male and Female, Age varies)

Foster care offers children a safe, nurturing home environment, while Specialized Foster Care provides an alternative to institutional care for youth unable to return to their families.

ADOPTION & POST-PERMANENCY

(Males and Female, Age varies)

When reunification isn’t possible, adoption offers a path to permanency. Support is often needed to help children and families adjust to their new life together.

ACUTE PARTIAL HOSPITALIZATION

(Males and Female, Ages 5-18)

A structured, short-term mental health program providing intensive therapy and stabilization for youth ages 5-18 experiencing severe emotional or psychiatric challenges, offered five days a week, year-round.

IN-HOME SERVICES

Our In-Home Program provides therapeutic care and counseling for at-risk youth, addressing mental health, behavioral issues, and family conflicts. Through strengths-based support, we help youth build healthier futures, strengthen family relationships, and reduce the risk of future offenses.

NURSE FAMILY PARTNERSHIP - BERKS/SCHUYLKILL

(Serving first-time expectant mothers through their child’s second birthday)

Nurse-Family Partnership is an evidence-based home visitation program that connects first-time mothers with specially trained registered nurses. Nurses provide ongoing support throughout pregnancy and the early years of parenting to promote healthy pregnancies, improve child health and development, and help families become economically self-sufficient. Services are personalized, empowering parents to build a strong foundation for their child’s future.

PARENTS AS TEACHERS - READING (BERKS)

(For families expecting or parenting children from birth through kindergarten)

Parents as Teachers is an evidence-based home visitation program delivered by trained Parent Educators with backgrounds in Early Childhood Education, Human Services, or related fields. The program offers personalized home visits, developmental screenings, group activities, and connections to community resources to support school readiness and family well-being. Bilingual staff are available to serve Spanish-speaking families.

(For adults living with mental health or substance use challenges)

Circle of Friends is a Drop-In Center that provides social, recreational, and supportive services for adults age 18 and older. The program offers meals, counseling, life skills training, and peer support in a warm, community-focused environment that promotes connection, recovery, and wellness.

ELECT - BERKS

(Supporting pregnant and parenting teens under age 21)

The ELECT (Education Leading to Employment and Career Training) program supports pregnant and parenting teens who are enrolled in school or seeking to re-enroll. The program helps young parents stay on track academically while addressing their unique needs. CPP operates the program with two dedicated case managers—one based at Reading High School and another leading the Fatherhood Initiative throughout Berks County. Program outcomes are monitored and reported by BCIU to ensure quality and compliance.

PROJECT PEACE

(Students ages 10-18 in the Reading School District)

Project PEACE is a youth-led initiative formed in 2011 in response to the tragic loss of four Reading students. Made up of middle and high school students, the program aims to curb youth violence and drug use by educating peers and promoting peace throughout the community.

KIDS AGAINST PRESSURE - READING (BERKS)

(Serving 3rd–5th grade students in the Reading School District)

Kids Against Pressure (KAP) is an after-school program designed to help students build character, resolve conflicts, strengthen self-esteem, and develop positive social skills. The program operates weekly in six classrooms and also offers a two-week, full-day summer camp each June.
